Successful Field Education:
- is built upon a knowledge base reflected in the required courses;
- is an opportunity to practice Social Work at a beginning level in the Social Work field under weekly individual supervision
- provides a 12-credit hour, 4000-level course requiring a high degree of self-direction and personal initiative.
Approval for Field Education includes the submission of this application to the Field Director and acceptance for Field by the Social Work Faculty Evaluation Committee. This application will be evaluated on the following four components. Each component is worth 10 points. A minimum score of 30/40 is required.
- quality of application
- record of grade achievements in practice courses (attach the program evaluation)
- articulation of professional goals and growth
- evidence of commitment to the NASW Code of Ethics
Students must satisfactorily complete all required Social Work courses prior to entering Field Education. Please include a resume and the current Program Evaluation from Web Advisor. Students who have any grade below a “C+” in the required Social Work practice courses (SWK 3350, 3351, 3352, and 3353) are required to address the deficits with the Director of Field. Field sites may be restricted for students with any grades below “C+”. * FYI: Student QPA’s can be found on Web Advisor.
If faculty have concerns regarding a student’s ability to function in a Social Work practice setting, these concerns will be shared with the student by the Field Director. A decision will be made in a timely manner by the Social Work Faculty Evaluation Committee and notification given regarding the application and readiness for Field Education.

- What is one skill developed in a practice classes that will be most beneficial in Field Education?
- What has been learned in practice classes that has enhanced a continued commitment to the National Association of Social Worker’s Code of Ethics?
- Will the existence of a criminal record impact an ability to intern in certain Field sites? If so, please indicate the charges or record.
- (Optional) In order to match students with appropriate Field Education sites, it is critical to understand each student’s needs and how best a specific placement would be of benefit. If there is information that will help the Field Director to explore Field Education options, please feel free to discuss this with the Field Director or note it below. Examples of this information may be physical limitations, learning disabilities, transportation problems, or specific concerns that should be considered.
- Discuss qualifications for practice including the ability to establish and sustain a relationship, deal with diversity, and show an understanding and concern about social and economic justice.
- Personal goals: Write a brief description about the type of agency in which there is the most interest. In five years what personally will the student be doing in the field of Social Work?
